公证材料翻译整理——香港改名契

Deed Poll（改名契）：香港人要改名，要到入境处「人事登记处」进行身份证改名手续。如要改名的人士未满18岁，便需由父或母提出。但过程中入境处会要求申请改名的人交出有效的「改名契」。

下面介绍 Deed Poll 的常见模板，后附参考译文。
1 若更名人士为成年人：直接由本人办理
BY THIS DEED I, ZHANG, SHAN （张山）("my New Assumed Name") of Room XXX, King Fat House, Cheung Fat Estate, Tsing Yi, New Territories, Hong Hong, formerly known as ZHANG SAN (张三)("my Former Name"), holder of Hong Kong Identity Card No. M000000(0) and born on 1st January 1990 do hereby renounce and abandon the use of my Former Name and in lieu thereof and do assume from the date hereof my New Assumed Name.

And in pursuance of such change of name as aforesaid,I HEREBY DECLARE that I will at all times hereafter in all records, deeds and instruments in writing, and in all actions, proceddings, and in all dealings and transations, and upon all occasions whatsoever use and sign my New Assumed Nme as my name in lieu of my Former Name so renounced as aforesaid.

And I hereby authorise and request all persons to designate and address me by my New Assumed Name only.

IN WITNESS WHEREOF I have hereunto set my hand and seal this 9th July 2016.

知识要点归纳
1. sb of +居住地址：表示某人在该地址居住；
        sb of +工作地址：表示某人在该地址工作；
2. natural parents （= biological parents）：亲生父母
3. former name：曾用名
4. do hereby + 动词原形：在公证文书中常见，表示“特此……”，如：do hereby certify：特此证明
do + 动词原形：表示强调。如：Do remember. （请务必记得。）
5. address/designate sb by/as：以……称呼某人
6. set one's hand and seal：签名和盖章
另外：公证文书或合同协议末的 Given under my hand and seal this 10th July 2016. 意思是：2016年07月10日经本人签名和盖章。
7. 特别注意 We have hereunto set our hands and seals this 9th July 2016 中的 this。在公证文书常见的是 this 而不是 the，表示“当天”。
